The following is an agreement between Starlight Trust Animal Rescue, henceforth known as STAR, and the above named, henceforth known as The Guardian.

The Guardian agrees to

· care for the animal(s) in accordance with STAR's published care sheets and treat them in a humane manner at all times.

· seek prompt veterinary attention for the animals if sick or injured.

· take all steps to prevent the animal from breeding. The Guardian further agrees to surrender any offspring that result from any accidental mating to STAR.

· keep the animal(s) only as a personal pet, not to be used for vivisection, feeding of other animals or any other purpose.

· never use softwood bedding such as sawdust or wood shavings in the animals habitat

· provide the animal(s) with adequate attention and opportunity to free range outside of the cage, spending at least half an hour a day interacting with the animal(s).

· return the animals to STAR should they be unable to care for them or abide by all the terms of this contract. The Guardian agrees not to relinquish custody of the animal except to return the animal to STAR.

· keep in touch with STAR and notify them of any change in address or phone number and to provide updates on the animal(s). The Guardian further agrees to notify STAR upon the death of any of the animals.

· allow STAR the right, by prior arrangement, to inspect the animals' new home at any time during their lifetime.

· keep the animal(s) safe from harmful objects and any other living creatures which may endanger the animal(s) safety.

· only have the animals euthanased at the recommendation of a veterinary surgeon for medical reasons.

The Guardian acknowledges that

· the animal(s) are entrusted to their care but remain the legal property of STAR.

· STAR shall not be held liable for any damages caused by the animal(s) to property, person or other pets. If any medical problems should occur STAR will be available for advice but is not liable for any medical bills. If The Guardian is unable or unwilling to pay these bills, the animal(s) must be returned to STAR for treatment.

· this contract affords STAR the right to immediately repossess any animal(s) should they have reason to believe that the terms of this contract have been breached.

· any donation given at the point of adoption is to support the ongoing work of STAR. It is not in purchase of the animal and all donations made are non-refundable.

The Guardian represents that

· neither they or any person residing in the household have been subject to legal action for cruelty to or neglect of animals or have had any animal seized by an animal welfare organisation for welfare reasons or breach of contract.

STAR agrees to

· accept the animal(s) back into their care at any time for any reason, relieving The Guardian of any ongoing responsibility for the animals care.

· remain available to The Guardian for advice as needed for the life of the animal.
